What is Family Connects?

All babies deserve the healthiest possible start. Family Connects Oregon is a no cost, evidence-based, nurse home visiting program for all families with newborns.

The Family Connects Oregon program supports parents by linking nurses, community resources, and families. Our mission is to connect you with resources that nurture your whole family and support your child. It’s about planting seeds for the future. Together, we’re growing healthy babies!

Family Connects Oregon is provided to all families with a newborn in Douglas County through United Community Action Network (UCAN).

Eligibility:

  • Live in Douglas County

  • Caregiver of newborn baby, including foster and adoptive caregivers

  • Accepting all insurance including OHP/Medicaid and Private/Commercial insurance plans

best gift you can give yourself!

Family Connect Nurses partner with families to provide:

  • 1-3 post-partum visits starting at around 3 weeks of age

  • Maternal Physical and Mental Health Assessment

  • Infant Health Assessment

  • Lactation/feeding support

  • Postpartum and Newborn education

  • Linkages to needed resources in the community
