Family  Connects Central Oregon provides Public Health Nurse home visits to  families in Central Oregon who are welcoming a new baby.  A visit is  scheduled between birth and your first doctor’s visit and assesses mom  and baby health, answers questions you may have and provides connection  to resources.

Visits are provided at no cost to families and are a continuation of your medical care.
